You will be responsible for assisting the RN Case Manager in carrying out the plan of care for your assigned patients on an intermittent basis in their place of residence. Recognize and address the impact of social and economic factors on patients, ensuring their emotional and physical comfort and safety, as well as that of their families. Document patient visits by preparing clinical and progress notes. Conduct ongoing assessments during each visit and accurately document findings. Observe, record, and report patient symptoms, reactions, and any changes to the appropriate healthcare provider. Support the Registered Nurse in performing specialized procedures as directed and by physicians orders. Participate in on-call responsibilities as outlined by the on-call policy. Perform other duties as assigned. Qualifications: Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia At least one year of hospice experience is required. Graduate of an accredited nursing school. Must have a valid driver's license and a reliable car available during work hours.
