I. PRIMARY FUNCTION/GENERAL PURPOSE OF POSITION :
The Clinical Coordinator is a Registered Professional Nurse with
twenty-four hour responsibility and accountability for the
coordination of clinical activities for a given team. Develops and
sustains an environment that promotes excellence in clinical
practice and customer satisfaction, while striving for optimal
efficiency and productivity of all resources.
The Clinical Coordinator is an advanced unit-based clinician who,
with increasing independence, utilizes the nursing process and
current clinical knowledge to provide professional nursing care for
all patients on the nursing unit. Utilizes advanced knowledge and
experience to assess the health needs of patients, collaborate with
health team remembers to coordinate patient care, anticipate the
outcomes of the nursing interventions, and set criteria for the
quality of patient care. Provides patient and staff education,
clinical supervision, and assesses need for and implements unit
based activities (quality assessment, in-services, etc.). Accepts
responsibility and accountability for ethical/legal functioning.
Participates in unit management activities (i.e. scheduling, budget
issues, counseling), provides supervision for staff as needed, and
assumes increasing leadership responsibility for professional
practice on the unit. May act in Administrative Director capacity
in her/his absence.
EMPLOYMENT QUALIFICATIONS :
Must be a graduate of an accredited school of nursing.
Must be currently licensed as a Registered Nurse in the State of
Virginia.
Must possess knowledge of hospital policies and nursing policies
and procedures and techniques within assigned patient care
area.
Cumulative experience must be a minimum of two years of acute
care.
Minimum of one-year experience in the Care Center. Must maintain
current CPR certification. Must have broad based knowledge of
clinical, psychosocial, and patho/physiological theories relating
to nursing diagnosis and treatment.
Possesses the ability to perform the range of nursing techniques
found
in general nursing practice including the unusual and non-routine.
Utilizes teaching skills and understanding of group dynamics.
Must have basic knowledge of management theories (i.e. financial
management, supervision, leadership).
Responsible for overseeing staffing levels on a shift-to-shift
basis, and more long term schedule planning. Arbitrates when
necessary to ensure adequate staffing levels are maintained.
Oversees bed placement of patients.
Continually evaluates patient care, environment and staff members
to achieve center goals and targets. Makes frequent rounds
encouraging staff to focus on clinical pathways and the
patient's
individualize plan of care.
Assists staff with active case management of patients.
Visits, interviews, and observes patients.
Plans, implements, evaluates, and modifies independent professional
and/or nursing interventions related to the disease process and
patient/family/significant other needs.
Oversees patient care assignments.
Collaborates with other members of the health care team to plan
patient care outcomes and modifies the plan of care as necessary.
Performs CPR as needed. Coordinates care delivery to achieve
optimal patient benefit, e.g. scheduling of tests and
activities.
Monitors medical record for completeness and evidence of
documentation by exception. Serves as a resource to staff for
evaluation of complex or unstable patients. Facilitates
availability of appropriate team leader for physician rounds or
team member for interdisciplinary collaboration of care.
Facilitates multidisciplinary and/or discharge rounds.
Assesses the unit and patient environment for general maintenance
and potentially hazardous conditions and takes corrective
action.
Demonstrates the knowledge and skill necessary to provide
age-appropriate care to patients served. Demonstrates knowledge of
the principles of growth and development of the life span
and possesses the ability to interpret data reflective of the
patient's status.
m. Provides input into the development of peers and staff by
demonstrating the ability to critically assess the level of
practice on the nursing unit and recommending appropriate
changes.
Identifies quality assessment indicators and assists in tracking
patient outcomes. Collaborates with Projector Coordinator to access
appropriate data to disseminate to team members for the purpose of
meeting outcome targets of cost, quality, and service. Utilizes
clinical experience and physical assessment skills to delineate and
define the physiological status of the patient. In addition,
utilizes these skills to assess the educational, psychological, and
spiritual needs of patient/significant other/family. Seeks
pertinent additional information related to the disease process and
patient/significant other/family needs from available
resources.
Collaborates with appropriate parties to coordinate educational
efforts for staff and patients.
Oversees recordkeeping to ensure completeness.
Participates in the education of team members.
Advises and teaches patients/families individualizing information
and teaching strategies. Coordinates the Human Resources aspects of
the team.
Distributes and retrieves evaluation materials to/from staff
responsible for completing them. Assures that evaluations are
submitted to Human Resources in a timely manner. Assists staff in
utilizing the peer evaluation process.
Evaluates team member's performance in an ongoing manner.
Participates in the interview process and educates team members on
interviewing skills. Utilizes counseling and disciplinary action
when appropriate.
Utilizes a variety of approaches to assist staff in becoming
self-directed work teams. Fosters and encourages open communication
and conflict resolution among staff members. Utilizes various
leadership styles based on assessment of staff's developmental
level. Promotes critical thinking and effective problem solving in
staff through example and reinforcement.
Assists in identifying and removing non value-added activities.
Assists team leaders to develop skills for appropriate coordination
and delegation of care. Assists in education of team members to
foster monitoring of resource utilization. Utilizes SMH standards
of practice and professional practice standards (ANA, AACN, NAACOG,
NASW, VASAP, etc.) and SMH Nursing Department Standards of
Professional Behaviors, in the delivery of care and for
self-assessment; holds others accountable to utilize the same
standards. Exhibits positive customer relations and communication
skills with staff, physicians, patients and families, ancillary
departments and community.
Works with preceptors to ensure an effective orientation and
positive learning and social experience for new staff. Coordinates
clinical rotations with instructors for students affiliating with
SMH. Utilizes and hold staff accountable for internal and external
professional practice standards. Applies effective conflict
resolution skills. Demonstrate cultural awareness. Communicates
with other departments in a positive manner while holding them
accountable.
Supports new initiatives and changes and assists staff in
understanding rationales. Involves staff in planning processes
which affect them. Identifies areas for Continual Improvement with
regard to cost, quality and service. Positively manages change by
adapting to the demands of the situation.
Plans self and staff educational development based on identified
needs. Collaborates in the development and implementation of unit
and departmental education. Advices, precepts and teaches staff.
Assesses equipment needs of the unit; meets those needs by
utilizing intrafacility resources. Serves and intrafacility
resource for patient and peer in-service educational needs.
Observes staff for effectiveness in emergency situations.
Performs CPR as needed.
Maintains center-specific certifications relative to the population
served, i.e.ACLS, PALS, etc. Facilitates removing of remains to
morgue, ensuring that appropriate documentation has been completed.
Accepts responsibility and accountability for ethical/legal
functioning; integrates knowledge of
ethical and legal concepts in decision making. Seeks support and
resources regarding ethical issues e.g. Pastoral Care, Ethics
Committee. Alerts Administrative Director or Risk Manager to
potential liability situations.
Serves as the team resource and assures completion of computerized
programs including possible application of: One-Staff and
KRONOS.
Incorporates into practice current knowledge of nursing education,
trends and new developments in the health care field. Maintains
mandatory unit certifications. Identifying deficiencies and seeking
opportunities to correct them. Interprets patient data from
multiple sources and selects and implements the appropriate
interventions for a select group of
patients on the nursing unit. Identifies the need for and assists
in the acquisition of in-hospital and post-discharge resources for
the patient and family. Will actively demonstrate excellence in
service behaviors to increase patient satisfaction/PRC scores. Will
participate in the Gallup Impact Plan for department to improve
work environment. Demonstrates the knowledge and skills necessary
to provide care appropriate to the age of patients served on his or
her assigned unit. The individual must demonstrate knowledge of
the
principles of the growth and development of the life span and
possess the ability to assess data reflective of the patient's
status and interpret the appropriate information needed to identify
each patient's status and interpret the appropriate information
needed to care needed as described in
the departmental policies and procedures. Provides Patient Care
based on specific age group on assigned unit:
6W/PICU: Neonate, pediatric
3E, 3W, Nursery, L&D: neonate, adult
NICU: neonate
ED: neonates, pediatrics, adolescents, adults and geriatrics
2N, 3N, 5E, 5W, 6E, 7W, PTU, PCVU, NSTU, ICU, CCU, CVICU:
adolescents, adults,
geriatrics
Must be able to lift, push, and pull up to 50 lbs. and perform all
functions involved in CPR.
