Position Summary
Functions as a mentor and resource role for the clinical team,
ensuring compliance with all TJC, CMS, BSMH, State and Federal
regulations. Acts as a resource to the Home Care staff in matters
pertaining to clinical, regulatory, and policy issues - including
but not limited to assigned DNBs within the EMR, revision work
queues, recert and discharge completion and timeliness,
participation in EMR meetings, team meetings, case conferences and
education. Manages Face to Face compliance and work queues.
Essential Functions
Responsible for and manages Face to Face compliance across
agency.
Participates in the agency Quality Improvement initiatives,
including the oversight and ownership of Performance Improvement
Plans as assigned to assist the agency in achieving performance
metric and goals.
Demonstrates sound clinical judgement, decision-making, and
clinical guidance in interdisciplinary teams.
Responsible for completing daily EMR Work Queue review and
monitoring of timely clinical documentation per agency policy.
As needed participates in interdisciplinary team meetings and case
conference in collaboration with the Clinical manager or Director
of Home Care and may perform joint visits (Discipline and skill
appropriate) to monitor for regulatory compliance and
performance.
Participates as a team member by performing additional assignments
not directly related to the job description when workload requires
and as requested by supervisor.
Represents, demonstrates and encourages behaviors that embody the

Education
BSN preferred from an accredited school of nursing
Licensing/ Certification -
Virginia Department of Health RN licensure.
Minimum Years and Type of Experience:
Minimum of 3 years of Home Health Care experience
Other Knowledge, Skills and Abilities Required:
Strong knowledge of the Home Health Joint Commission standards and
the Medicare Conditions of Participation. Excellent clinical, oral
and written communication and interpersonal skills; OASIS accuracy,
Strong customer service skills.
