PHYSICIAN ASSISTANT/NURSE PRACTITIONER - EXTENDERSite Of
Service* Naval Medical Center Portsmouth, VA (NMCP) and its
associated Branch Health/Medical Clinics (BHC) and Tricare Prime
Clinics (TPC). * HCWs shall receive notification 48 hours prior to
reassignment with a forty (40) mile commuting radius of their

Qualification/Requirements* Degree/Education: * Physician
Assistant: Possess a Baccalaureate degree degree from a Physician
Assistant Program accredited by the Accreditation Review Committee
for the Physician Assistant (ARC-PA) of the Commission on
Accreditation of Allied Health Education Program (CAAHEP).* Nurse
Practitioner: Possess a Master's, Post-Master's or Doctorate degree
from an advanced practice nursing program accredited by the
Accreditation Commission for Education in Nursing (ACEN), formerly
known as the National League for Nursing Accrediting Commission
(NLNAC), or the Commission on Collegiate Nursing Education (CCNE).*
Experience: One year of full-time experience within the last two
years as a nurse practitioner or physician assistant in a primary
care or family practice setting. * Certifications: * Physician
Assistant: Certified by the National Commission on Certification of
Physician's Assistants (NCCPA). * Nurse Practitioner: Possess
current certification as a Nurse Practitioner by the American
Nurses Credentialing Center (ANCC) or American Academy of Nurse
Practitioners (AANP* Possess current Basic Life Saver (BLS) hands
on certification through the AHA.* Obtain Advanced Cardiac Life
Support (ACLS) certification prior to contract start and maintain
according to ACLS guidelines.* Licensure: Possess current
unrestricted license to practice as a Physician Assistant,
Registered Nurse, or Advanced Practice Nurse in one of the fifty
states, the District of Columbia, the Commonwealth of Puerto Rico,
Guam, or the U.S. Virgin Islands.* U.S. Citizenship: HCWs
performing under this contract shall be U.S. citizens.* English
Language Requirement: The Contractor shall ensure that all HCWs
providing services under this contract are able to read, write, and
speak English well enough to effectively communicate.* HCW Physical
Capability: HCWs shall be physically capable of standing and/or
sitting for extended periods of time and physically capable of
performing all services required under the contract and TO.*
Training Requirements: Attend the Annual Navy Pain Symposium/NCPMP
meeting. Date TBDPhysician ExtenderSPECIFIC DUTIES:* The HCW shall
provide services within the scope of the clinical privileges
granted by the commanding officer, the applicable duties provided
in the basic contract, and the following: * Become familiar with
the Military/Medical Treatment Facilities formulary. Providers
authorized to prescribe pharmaceuticals shall do so according to
the availability of drugs listed therein.Successfully complete the
MTF provided in-services in IV placement.Physician Extender
(PDHRA)* The HCW shall provide services within the scope of the
clinical privileges granted by the commanding officer, the
applicable duties provided in the basic contract, under 5.5.3.2.2.
and the following: * When assigned to the Directorate of Primary
Care and Branch Clinics, the HCW may be utilized for walk-in or
other scheduled appointments in support of other MTF clinics only
when all other PDHRA and deployment health commitments have been
fulfilled. * Ensure that all service members that require follow-on
appointments or referrals will have them before leaving the
clinic.* Become proficient with the use of the Navy and Marine
Corps Public Health Center electronic methods of completing the
Department of Defense (DD) Forms - An electronic or web-enabled DD
Form 2900 (or Standard Form 600 if electronic or web-enabled access
is not available) shall be utilized for post deployment health
reassessments, an electronic or web-enabled DD Form 2795 shall be
utilized for pre-deployment assessments, and an electronic or
web-enabled DD Form 2796 shall be utilized for post deployment
screenings. The HCW shall use the Standard Form (SF) 600 which is
the primary form of documentation for medical encounters, and the
(SF) 513, which is the primary consultation form.Physician
Assistant Duties/Nurse Practitioner Duties* Perform Deployment
Health screenings, assess the results, treat patients and refer
patients for further treatment as appropriate. Web-enabled Periodic
Heath Assessment and/or DoD directed health screenings shall also
be utilized. * Follow MTF specific referral/admission protocols,
when seeing patients that requires an admission to the MTF. The HCW
will not be privileged for hospital admissions nor will they have
after-hour on-call duties. The contract practitioners will not be
responsible for obstetrical care of patients.* Physician Extender
(MED HOME PORT): The HCW shall provide services within the scope of
the clinical privileges granted by the commanding officer, the
applicable duties provided in the basic contract, under 5.5.3.2.2.
and the following:* Successfully complete the MTF provided
in-services in IV placement, prior to assignment to this task.*
Perform Deployment Health screenings, assess the results, treat

of patients.* Medical Home Port (MHP) Duties, as assigned, include
the following:* Provide comprehensive health care for all ages and
families in their Medical Home Port Team, but particularly their
assigned patients, utilizing evidenced-based clinical practice
guidelines and engage patients actively in their health care.*
Provide health maintenance and preventive services to individual
patients as recommended by accepted guidelines. Engage in proactive
population based health management, providing regular surveillance
to patient's need for preventive services and management of chronic
disease leveraging information technology decision support tools
and the members of the medical home port team.* Proactively
collaborate and integrate with members of the team, other
departments and directorates to achieve optimal care for the
patient in the HOME PORT model. * Actively participate in MHP Team
huddles, proactively assist team's identification of diagnostic
procedures required during schedule scrub, and assist with
identification of patient education needs and review plan with team
members for coordination of educational needs.* Engage other MHP
Team members to assist in improving documentation of the episode of
care using MHP Clinic-approved templates as the initial template
(ie. TriService Workflow/COMPASS) - Reason for today's visit,
History of Present Illness, Past Medical and Surgical History,
Social History, and Medications with Medication Reconciliation.*
Document accurately, timely and completely in the patient's
electronic medical record (AHLTA) (and hard copy when required),
providing appropriate code level and diagnosis of patient visits
using CPT and IDC-9 coding standards* Communicate to patients
utilizing asynchronous messaging and telephone consults.*
Participate in training Navy Corpsman in delivering primary care to
prepare them for deployment forward in support of overseas
contingency operations.* Precept students (residents, NP, PA, IDC
etc.) and new staff members as directed.* Assist in the
professional continuing education of development of other staff
members as appropriate including, students (residents, NP, PA, IDC,
etc), peers, colleagues, paraprofessionals and other health
professionals.* Train and exercise technical direction over
assigned professional staff in MHP Teams.Hours* Physician Extenders
(BHC NAVSTA Norfolk) - Services shall be provided Monday through
Friday, between the hours of 0600 and 1800 hours. The HCW will be
scheduled for a shift within the time range depending on patient
volume and clinical scheduling needs. The shift lengths could range
from eight (8) to twelve (12) hours. Individual shift hours may
occasionally be extended to accommodate travel team requirements or
mass screening evolution, however in no instance will the shift
exceed twelve (12) hours in length. Shifts will normally be
scheduled for an eight and half (8.5) to twelve (12) hour period,
to include an uncompensated thirty (30) minute to sixty (60) minute
lunch break, respectively. The HCW may be required to occasionally
work a Saturday shift to accommodate the deployment training
schedule. The requirement for working on a Saturday will not exceed
twelve (12) Saturdays per year. In no instance will the HCW be
required to provide services in excess of eighty (80) hours per
two-week period. The clinic is open on the following Federal
Holidays: Martin Luther King Jr's Birthday, President's Day,
Columbus Day, and Veteran's Day. Services shall not be required on
federal holidays when the clinic is open.* Physician Extenders (BHC
Boone) - Services shall be provided Monday through Sunday between
0700 and 1900 for one hundred forty-nine and half (149.5) hours per
week. The clinic is open on the following Federal Holidays: Martin
Luther King Jr's Birthday, President's Day, Columbus Day, and
Veteran's. Weekend and holiday services may be required on a
rotational basis at NMC Portsmouth. Upon mutual agreement between
the HCW and supervisor, compensatory time off will be granted for
weekend and holiday services. * Physician Extender (TPC Virginia
Beach) - Services shall be provided Monday through Sunday between
0700 and 1900 for one hundred ninety-six (196) hours per week.
Requirements for services may vary on holiday weeks. The government
provided matrix shall reflect these changes. Clinic is open on all
Federal Holidays. * Physician Extenders (TPC Suffolk) - Services
shall be provided Monday through Sunday between 0700 and 1900 for
one hundred twenty (120) hours per week. The clinic is open on all
Federal Holidays.Orientation Hours* Each Physician Extenders shall
attend a total of 44 hours of orientation, as follows: eight (8)
hours of Command Orientation, twenty-four (24) hours of Work Space
Orientation, and twelve (12) hours of CHCS & AHLTA Orientation.
Special Requirements/SkillsMust be comfortable in a fast-paced,
dynamic environment. Must be able and willing to reprioritize on
short notice and work on multiple simultaneous projects. Flexible
and able to work with various personalities. Team work skills
required. Time management skills required. The ability to meet
deadlines in a deadline intensive environment is essential. High
level of adaptability and willingness to embrace change in a
