Thank you for considering a career at Bon Secours!PRIMARY FUNCTION/GENERAL PURPOSE OF POSITION:The Care Manager (CM) provides coordination of care for the patient through facilitation/communication and discharge planning throughout the continuum of care.The CM is a key contributor to the overall financial, quality, and clinical performance. The CM supports an outcomes-oriented patient care delivery system, which places the patient at the center of all activities. The CM assesses all patients on the rehab unit upon admission to identify needs for discharge and works with the treatment care team, the patient, and the family (or primary caretaker) to establish an acceptable discharge plan of care. The CM is a resource for the patient and family to obtain the necessary resources that they may need regarding their care, treatment, and discharge.The CM works with patients to ensure social and psychosocial needs are met, assists with eligibility determinations for social programs and funding sources, and ensures access to community assistance programs if so, needed by the patient. A CM provides a positive financial impact to the organization through extensive interaction with physicians, nurses, and other patient care givers in managing length of stay and limited readmissions. The CM offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of an effective discharge plan of care for all patient populations. The CM also works with the care team to fulfill the goals of the health system by controlling length of stay and readmissions and following isolation protocols to decrease the hospital spread of infection.EMPLOYMENT QUALIFICATIONS: A Clinical Social Worker (BSW, MSW or LCSW) or a Licensed Registered Nurse (Licensed as an RN in the Commonwealth of Virginia), preferably with a Bachelors Degree or higher, or at least three years previous experience in a healthcare/clinical setting preferred. If a BSW, MSW, or LCSW, employee must have at least two years of experience in the case management field. If a Licenses RN, prefer that employee have at least three years of clinical experience. If employee has received a license in a clinical setting previously, primary license verification required. If previously licensed, prefer that candidate have had at least 3 years of experience in a healthcare/clinical setting. The employee must have strong interpersonal communication skills with an ability to work independently. The employee must have strong organizational and time management skills as evidenced by a capacity to prioritize multiple tasks and role components. Must have or complete the Case Management Certification (ACM or CCM) within three years of employment.ESSENTIAL JOB FUNCTIONS:Develops, coordinates and monitors strategies for obtaining, organizing, and allocating human and material resources required for acute rehab d/c transition.Establishes collaborative practice with nursing personnel, physicians, and health care professionals regarding the plan of care.Identifies variances in plan implementation.Identifies necessary resources or eliminates unnecessary resources needed to achieve quality patient outcomes.Manages pathway and non-pathway patients with a proactive, systematic approach.Receives and processes discharge plans and helps to facilitate a smooth transition into the home care network or other appropriate healthcare setting.Assesses appropriateness of referral based on Federal and State regulations, client eligibility for service, the patients home care needs and the ability of the agency to safely provide the needed services.Familiar with a readmission reduction scoring to establish patients with the highest risk for readmission.Assists with identification of alternative options for services if/when the patient does not meet criteria or if services are not available.Collaborates with health care team, develops treatment goals and plans interventions that assist the patient and family to resolve social and psychosocial concerns that may arise as a result of illness, engagement in high-risk social behaviors (i.e., substance abuse), limited finances, and lack of family support and/or physical disabilities.Involves patient/family in the planning process and educates them regarding continuity of care.Collaborates with post discharge care providers in the development of transition plans. Obtains physician involvement and agreement with plan. Advocates for patient choice.Proactively identifies and creatively problem solves barriers to access of community services and identifies gaps in the continuum of care.Successfully advocates access to community resources for patients who do not specifically meet eligibility requirements.Provides leadership for multidisciplinary team efforts during Interdisciplinary Rounds (IDRs).Understands data relative to patient population and utilizes information to improve quality and reduce cost.Works collaboratively with peers to achieve department goals and daily work as evidenced by appropriate and timely communication which is respectful and clear. Sensitive to workload of peers and shares responsibilities, fills in, and offers help.Actively participates in department process improvement teams, planning, implementation, and evaluation of activities.OTHER JOB FUNCTIONS:Actively participates in conferences, committees, and task forces as directed by Rehab Manager.Accepts additional assignments as assignedWORKING CONDITIONS:Office and clinical setting.
